(1) If the Contractor identifies or is notified by any source, (including a subcontractor at any tier), that any product or service provided or used (or to be provided or used) during contract performance does not comply with any prohibition in this clause, then the Contractor shall report the following information, or as much information is known, in writing to the contracting office as identified in paragraph (h)(2) within 72 hours: (i) Contract number and order number, if applicable; (ii) The specific prohibition the product or service is not complying with; (iii) A description of the products or services that the Contractor identifies or has reason to suspect is prohibited (include brand; model number, such as the original equipment manufacturer (OEM) number, manufacturer part number, or wholesaler number; and item description, as applicable); (iv) The entity that produced the product or service (include entity name, unique entity identifier, Contractor and Government Entity (CAGE) code, facilities responsible for design, fabrication, assembly, packaging, and test of the product, and whether the entity was the OEM or a distributor (provide manufacturer codes and distributor codes used for the product)); 36C26026Q0995

This is a combined synopsis/solicitation for TCGRx Annual Maintenance & Support for VISN 20 in accordance with the Statement of Work as prepared in accordance with the format in Subpart 12.201-1(c)(2), as supplemented in the attached Request for Quotes (RFQ). A firm-fixed price purchase order is anticipated. Simplified Acquisition Procedures IAW FAR 12 will be used. This announcement constitutes only solicitation. The solicitation number is 36C26026Q0995 and is attached. The solicitation document and incorporated provisions and clauses are those in effect through: Federal Acquisition Circular: FAC - 2025-06 Effective Date: August 2025. This solicitation is unrestricted, and the associated NAICS Code 811210 has a business size standard of $34 Million. Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 12. Comparative evaluation is the side-by-side comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The award will be given to the most advantageous respondent to the Government. Responses should contain your best terms and conditions. Quotes must be emailed to: angela.williams3@va.gov and received no later than 9:00 am PST on 09/11/2026. Quotes may be submitted on the attached RFQ or the vendor s own form. All questions must be submitted in writing and will not be answered via telephone, no exceptions. RFQ responses that are incomplete will not be evaluated for consideration.
