Scope

36C25626Q1094 Page 9 of 46 • Current VA requirements governing reprocessing of reusable medical devices, including VHA Directive 1116(2), as applicable. • Applicable ANSI/AAMI guidance, including ST79 and ST91 when within the scope of the devices processed; the Contractor shall not represent that product ownership alone ensures compliance. • Applicable OSHA worker-safety and ergonomic principles, including reduction of excessive bending, reaching, awkward posture, and wet-area electrical hazards. • Current local plumbing, electrical, mechanical, building, accessibility, seismic, infection- control, and life-safety requirements. • Equipment manufacturer installation instructions and MIFUs for devices and chemistries used by SLVHCS. • Shall comply with all VA infection-control risk assessment and interim life-safety measures for the project.

Deliverables

Offerors shall provide in detail each specification that is required in the Schedule and Scope of Work.

Evaluation

The Government will award a Firm Fixed Price Contract resulting from the offeror whose quote conforms to the specifications of the solicitation included in the Price/ Cost Schedule, Scope of Work parts, Salient Characteristics, Delivery schedule, Contractor requirement, Manufacturer Warranty, and this solicitation and is the Lowest Price Technically Acceptable to the Government in accordance with FAR 12.6- Streamline Procedures for Evaluation and Solicitation for Commercial Items and is being conducted under FAR Subpart 13.5- Simplified Procedures for Certain Commercial Items apply.

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with format in Subpart 12.6- Streamline Procedures for Evaluation and Solicitation for Commercial Item. This solicitation is issued under authority of FAR 13.5, Simplified Procedures for Certain Commercial Items, and the conduct of the procurement will be in accordance with FAR Parts 12 and 13 or some combination thereof. The solicitation number is 36C25626Q1282 and is issued as a Request for Quotation (RFQ). The proposed contract action is for Triple Basin Reprocessing Sinks for the Southeast Louisiana Veteran Health Care System (SLVHCS), located at New Orleans, LA for which the Government intends to solicit competitive quotes. Interested offerors may submit quotes, and all eligible quotes must be received prior to the response date listed on FBO to be considered by the Government. This solicitation is 100% Service Disabled Veteran Owned Small Business (SDVOSB) set-aside. Applicable NAICS: 339113 Business Size Standard: 800 Employees This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2026-01). Contractor is to provide triple basin reprocessing sinks and accessories as detailed within this solicitation to Southeast Louisiana Veterans Health Care System (SLVHCS) located at 2400 Canal St, New Orleans, LA 70119.
