Scope

As prescribed in 23.304(a)(2), add the following paragraph (i) to the basic clause: (i) Except as provided in paragraph (i)(2), the Contractor shall prepare and submit a sufficient number of Safety Data Sheets (SDS's), meeting the requirements of 29 CFR 1910.1200(g) and the latest version of Federal Standard No.313, for all hazardous materials identified in paragraph (b) of this clause. (1) For items shipped to consignees, the Contractor shall include a copy of the SDS's with the packing list or other suitable shipping document which accompanies each shipment.

Solicitation no. 75H71026Q00179 is issued as a/an Request for Quotation (RFQ) for the provision of laboratory supplies, reagents, diagnostic test kits, collection supplies, quality control materials and related laboratory consumables for the Crownpoint Healthcare Facility Laboratory, The North American Industry Classification System (NAICS) is 325413 - In-Vitro Diagnostic Substances Manufacturing - 1250 employees This acquisition is a 100% Indian Small Business Economic Enterprise (ISBEE) set-aside under the Buy Indian Act. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov and www.acquisition.gov/hhsar. Offerors shall review the attached Combined Synopsis/Solicitation for complete requirements, submission insturctions, evaluation factors, and terms and condititions. Attachment 1 - Laboratory Supply Evaluation Schedule provides the representative items for quotation and evaluation, and Attachment 2 - Stattement of Work (SOW) provides the Government's requirements. The supply list is representative and not all-inclusive; order will be placed on an as-needed basis based on actual laboratory requirements. Brand Name: Unless otherwise specifically identified in the SOW or Attachment 1, manufacturer names and part numbers are provided for identification and evaluation purposes. Equivalent products may be offered, subject to Government determination of technical acceptability. The period of performance (or delivery) is: Base Period: October 1, 2026, through September 30, 2027 Option Period One: October 1, 2027, through September 30, 2028 Delivery shall be FOB Destination to the Crownpoint Healthcare Facility, Crownpoint, New Mexico 87313 Supplies shall be delivered within the delivery times proposed in the Contractor's quotation and accepted by the Government. DescriptionThe Indian Health Service, Navajo Area, Crownpoint Healthcare Facility, located in/at the Jct. of NM State Hwy 371 & Navajo Route 9, Crownpoint, NM 87313 has an upcoming requirement for laboratory supplies, and reagents on an-as-needed basis for the Crownpoint Healthcare Facility, Laboratory Department. The Government anticipates establishing a Blanket Purchase Agreement (BPA), or other appropriate commercial supply vehicle for the acquisition of laboratory supplies, reagents, specimen collection materials, controls, testing kits, consumables, and related laboratory products required to support patient care and diagnostic services. The contractor should be able to provide a full-line laboratory supply catalog and be capable of furnishing a broad range of laboratory supplies and reagents as required by the Government. Supplies will be ordered on an as-needed basis throughout the ordering period. The anticipated ordering period is: Base Period: July 1, 2026, through June 30, 2027 Option Period : July 1, 2027, through June 30, 2028. The North American Industry Classification System or NAICS code is - In-Vitro Diagnostic Substance Manufacturing with a small business size standard of 1,250 employees. The Request for Quotations No. 75H71026Q00179 in accordance with 12.201-1 will be issued on or by June 22, 2026. Evaluation factors may include technical capability to meet the government need, past performance, and price. Quotes will be due on or by June 26, 2026. The acquisition set-aside determination has not yet been made. The Government will use information received in response to this Sources Sought Notice to determine the appropriate acquisition strategy.
