Scope

An AQL does not allow a contractor to knowingly offer defective service but limits reduced payment to circumstances in which defective performance results in a measurable reduction in the value of services rendered. **2.1.2 Defective Service** A unit of service that contains one or more defects or nonconformance with specified requirements. **2.1.3 Lot** The total number of service outputs in a surveillance period, as defined in the AQL column of the PRS. **2.1.4 Performance Requirements Summary (PRS)** Identifies the key service outputs of the contract that will be evaluated by the Government to ensure contract performance standards are met by the contractor. **2.1.5 Quality Assurance (QA)** The actions taken by the Government to ensure that services meet the requirements of the PWS. **2.1.6 Contracting Officer’s Representative** A Government person designated by the CO with authority to act within the scope of the delegated authority. **2.1.7 Quality Assurance Surveillance Plan (QASP)** The purpose of the QASP is to describe the systematic methods used to monitor performance and to identify the required documentation and the resources to be employed.

Sources Sought: Radiology Interpretation & On-Site Services to be provided to Cheyenne River Health Center, located in Eagle Butte, SD. Sources Sought Notice Number: IHS1528235 This Sources Sough is to determine the availability of Indian Small Businesses Economic Enterprises (ISBEEs), Indian Economic Enterprises (IEEs), and other small businesses capable of performing the requirement. This is a Sources Sought notice. This notice DOES NOT constitute a request of proposals, request for quote, or invitation for bid. This Sources Sough is a market research tool utilized to determine the availability of qualified small business concerns prior to issuing a Request for Quote/Proposal. This is not a solicitation for proposal, and no contract will be awarded from this announcement. BACKGROUND Firm Fixed-Price, Non-personal contract to provide Radiology Interpretation & On-Site Services for the Cheyenne River Health Center, Eagle Butte, SD 57625 OBJECTIVE The objective of this requirement is to provide Radiology Interpretation & On-Site services. The award is to be a base year, plus four (4) option years. SCOPE See attached PWS CONTRACT REQUIREMENTS/ AND PERSONNEL QUALIFICATIONS See attached PWS TYPE OF ORDER This is a firm-fixed price contract. ANTICIPATED PERIOD OF PERFORMANCE Period of Performance: Base, plus four (4) option years. PLACE OF PERFORMANCE Cheyenne River Health Center 24276 166th St. Airport Rd. Eagle Butte, SD 57625 PAYMENT INVOICE SUBMISSION AND PAYMENT In compliance with the Office of Management and Budget (OMB) M-15-19 memorandum Improving Government Efficiency and Saving Taxpayer Dollars Through Electronic Invoicing directing Federal agencies to adopt electronic invoicing as the primary means to disburse payment to vendors.
